Please fill out and send back or print out and bring in on their trial appointment. Trial appointments are approximately 40 minutes long and give us a chance to assess your dogs suitability to our day care. We do ask that you, as owners, do not stay for these trials to give us the opportunity to assess your dog properly.

Please be reminded all dogs must be vaccinated and desexed after the age of 6 months. We will need to see evidence of both of these.
